

Ethicon provides cash contributions targeted to meet the needs of underserved populations. Contributions are provided to support specific programs and services of 501 (c) (3) non-profit organizations where measurable outcomes can be identified.
Special notice:
Please be aware that Ethicon will close out its 2010 cash contributions program to support specific programs and services of 501 (c) (3) non-profit organizations on July 31.
If you have a request for funding, please submit the request no later than July 31. Requests received after July 31, will be considered for 2011 funding.

Health:
As a medical device company, Ethicon’s charitable giving builds on our legacy of leadership and trust that our customers have in our people and products. We make contributions to programs that make life-changing, long-term differences in human health by Opening Doors and Restoring Lives.
Opening Doors:
- Improving the access and quality of affordable health care
- Building capacity of health care organizations to support direct treatment and care
- Advocating for and informing underserved populations about health care resources
- Encouraging individuals to pursue health care careers
Restoring Lives:
- Enabling patients and/or caregivers to take an active role in understanding their condition and managing their treatment and recovery
- Educating individuals about specific medical conditions
- Enabling patients to achieve greater normalcy in their lives
Requests for funding must reflect a program associated with a surgical intervention for a condition that impacts life or quality of life. It should also address at least one aspect of the surgical care path – the patient journey from the time diagnosed through surgical treatment, through restoration.
Our Local Communities:
Ethicon has a Credo responsibility to the civic and social health of the communities in which we have a presence. Our support reflects the needs of the local community and aligns with our identity as a leading corporate citizen. In locations where Ethicon headquarters or other facilities are located, we support select community responsibility initiatives that improve the quality of life for residents and the working environment for employees. These locations include Somerville, NJ, Raleigh, NC, Cornelia, GA, San Angelo, TX, San Lorenzo, PR, and Juarez, MX. The initiatives we support include programs and services in the areas of health and human services, education, the environment and arts and culture.
Special note regarding health related requests:
- Somerset/Hunterdon Counties, NJ: Ethicon participates in the Johnson & Johnson Somerset County Companies Healthcare and Basic needs Fund managed by the Community Foundation of New Jersey (CFNJ). If your organization is located in or serves residents in Somerset and Hunterdon counties in New Jersey and your request pertains to healthcare and basic needs, you are asked to view the program requirements and apply through CFNJ. More details can be found at www.cfnj.org.
- All other requests: Priority will be given to programs described under the Health category above. However, on a local community level, a broader scope of health care needs may be considered, based on the unique needs of the community.
Limitations
Ethicon does not extend cash contributions for:
- Individuals. Contributions are made only to qualified non-profit organizations.
- Trips or tours.
- Political, fraternal or athletic groups.
- Sectarian and religious organizations that do not serve the general public on a nondenominational basis.
- Building campaigns or endowments of any kind.
- Multi-year commitments for appeals for unrestricted funds.
Application Requirements
If you feel your request falls within these guidelines, you are invited to submit an online application for a cash contribution. Before beginning the online application you should be aware of the following important requirements:
Ensure your browser settings allow cookies.
You may start your application, save it and return later to continue working on it. Saved applications that have not been updated or submitted in 120 days will be deleted.
You will need your organization’s IRS 501 (c) (3) tax identification number.
You will be asked to upload the following required documents: (acceptable file types include pdf, Excel,and Word.)
- Your organization’s most recent audited financial statement
- List of board of directors or trustees. This list should only include name, title, and employer affiliation. (Do not include personal information such as address, phone number, etc.)
- Line item project budget
- Statement of non-discrimination in regard to employment practices
- Statement of non-discrimination in regard to population served
Our Review Process
Please note the following about our application process:
- Applications require a minimum of four weeks for review.
- Insufficient information may delay the review process. Additional information may be requested prior to a final decision.
- Requestors will be notified in writing of submission approval or decline.
